Differences in Social Determinants of Health contribute to the stark and persistent chronic disease disparities in the United States among racial, ethnic, and socioeconomic groups, systematically limiting opportunities for members of some groups to be healthy. Interventions targeting SDoH have tremendous potential to narrow disparities across many chronic diseases by removing systemic and unfair barriers to practicing healthy behaviors.

The health and economic crisis stemming from the pandemic has magnified the systemic barriers to health and how they are particularly worse for marginalized groups. Though past and current efforts have focused on addressing health outcomes – racial and geographic health disparities – today’s social climate demands that stakeholders acknowledge how systemic racism and economic inequality are drivers of health inequities, which, in turn, perpetuate disparities.

How can health plans, health systems and other healthcare stakeholders engage to improve health equity through targeted SDoH initiatives? The panel of speakers in this session provide case examples on providing optimal care through the lens of SDoH for black women residing in medically underserved communities. Through tech-enabled and community-based service providers involving transportation, virtual care, and prenatal and postpartum care, these collective uses of service combined with culturally competent providers and technology could have a resounding effect to improve outcomes and reduce global maternal mortality.

    Andrea Boudreaux,

    Dr. Andrea Boudreaux, FACHE

    Dr. Boudreaux is Board Certified in Healthcare Management as a Fellow in the American College of Healthcare Executives. Dr. Boudreaux is an accomplished health equity and public health leader, helping organizations to improve access to marginalized and historically underserved communities. She has been the Senior Partner at JCG Consulting Group, a minority owned firm based in New York, NY where she led the firm’s Manhattan office and provided leadership to the team on engagements requiring expertise in clinical integration; service line development; financial turnaround; organizational development; employee, patient and physician satisfaction improvement; executive team development; emergency management and disaster preparedness; Joint Commission accreditation support, and COVID19 response and recovery.

    In her time in clinical practice, Dr. Boudreaux worked to improve the health of children and adolescents through the integration of behavioral health and primary care. As the Executive Director of AGAPE, LLC, Dr. Boudreaux led a three-state expansion of a community mental health organization and provided care to thousands of patients annually. While she spent her clinical practice addressing mental health needs, Dr. Boudreaux has become a champion of maternal health and eliminating disparities for women of Color. Currently, Dr. Boudreaux serves as the Chair for the Healthcare Access and Quality Workgroup of the Mom and Baby Action Network, a partnership between the March of Dimes and the Centers for Disease Control.

     

    Alphonso Harvey

    Alphonso Harvey Jr. is a North Carolina born, Florida raised businessman based in Nashville, TN. With 15 years of corporate experience, he now serves as CEO of AAA DME LLC, a Licensed Home Medical Equipment Provider, focused on serving the needs of expecting mothers. Alphonso is an expert on patient acquisition and is extremely niche focused in his market strategy. Alphonso graduated with a Master’s in Business Administration from Regis University and an undergraduate degree from Vanderbilt University. Alphonso has received many accolades during his career including Top 40 under 40, to being an esteemed graduate of both SBA’s Scale Up Program, and CATALYST from Nashville’s Entrepreneur Center. 

     Alphonso is also very active in the community, volunteering with the HOYAS foundation, a group that mentors at risk middle school age boys. He also is very active in his fraternity, The Omega Psi Phi Fraternity Incorporated.  He is a Husband and father. He strives to help people achieve their dreams and subscribes to the mantra that, “There are no problems, just unfound solutions”.

    Mohamed Kamara embarked on a mission to affect a positive change in the healthcare ecosystem. After the passing of his older sister during childbirth due to a preventable condition, Mohamed was driven to come up with a solution to close the global gap in the healthcare industry. He took action by creating InovCares. InovCares is a behavior change platform for women, particularly women of color, that provides access and treatment to culturally-sensitive clinicians. We combine real-time metabolic health data gathering with telehealth, computer vision, artificial intelligence, patient engagement rewards, and a sense of community.

    Mohamed is also Show Host and Producer of The Empowered Patient Podcast, and had served as Chief Financial Officer for the Wellness Healthcare Clinic in the Washington DC metro area.

    Mohamed has an MBA from Mount Vernon Nazarene University and a B.S.B.A. in Accounting and Economics from the Ohio State University - The Max M. Fisher College of Business.

    Nathalie Occean

    Nathalie Occean is a New Jersey native and Rutgers New Brunswick alumna whose healthcare career began in Memphis, TN where she completed a Master's degree in Healthcare Administration at the University of Memphis while working full-time at the University of Tennessee Health Science Center. Post-graduate school, she relocated to Charleston, SC for an Administrative Fellowship in Healthcare Quality at the Medical University of South Carolina. At the conclusion of the Fellowship, Nathalie worked in Business Health where she managed service delivery to the health system's external employer partners to help businesses foster a culture of well-being within their workforces.

    She now works as the Chief Operating Officer for Memphis-based healthcare tech startup, MedHaul, focused on providing easy transportation solutions for people with special needs through their technology platform. This platform connects healthcare organizations to MedHaul's network of skilled non-emergency medical transportation providers to combat the challenges seen when patients miss appointments and care is delayed.

    Outside of work, Nathalie is actively involved with the National Association of Health Service Executives (NAHSE) supporting their mission of promoting the advancement and development of Black health care leaders, and elevating the quality of health care services rendered to minority and underserved communities. In her free time, she teaches tap dance with a focus on developing leadership skills while exposing children from underserved communities to the art form.
